Many individuals with chronic cervical pain present with muscle imbalances and reduced neuromuscular control, particularly involving the deep stabilizing muscles of the neck and shoulder girdle. These deficits contribute to the persistence of pain, diminished range of motion, and decreased quality of life. Rehabilitation through specific strengthening programs is therefore not only recommended, but essential.

A growing body of scientific literature confirms the effectiveness of cervical strengthening in reducing pain, improving posture, and restoring function. Whether we are dealing with post-traumatic cases, musculoskeletal overuse, or pain with a more complex origin like cervicogenic headache or temporomandibular dysfunction, the message is the same: strengthening is part of the solution.

The Foundations of Cervical Strengthening

It is important to understand that cervical strengthening involves more than just activating large, superficial muscles. In fact, one of the key therapeutic targets lies in the deep stabilizers—muscles such as the longus colli and longus capitis—which are often underactive or delayed in patients with neck pain. These muscles help stabilize the cervical vertebrae during movement and posture, preventing micro-instabilities that could otherwise perpetuate pain.

Another important component of rehabilitation includes the shoulder girdle. The close relationship between scapular positioning and neck biomechanics is well-established. Weakness or poor control in this area often increases mechanical stress on the cervical spine. That is why an effective rehabilitation program should address both regions: the neck and the shoulders, in a coordinated, progressive manner.

Who Can Benefit From Neck Strengthening?

Patients with Cervicalgia

Patients suffering from general neck pain, commonly referred to as cervicalgia, typically present with decreased endurance and poor motor control of the cervical musculature. For these patients, combining exercises that target cervical retraction with scapular stabilization is often beneficial. By reinforcing these muscle groups, we help reduce pain, improve posture, and promote long-term resilience against relapses.

Patients with Cervicobrachial NeuralgiaIn this condition, pain radiates from the neck into the arm, often due to nerve root irritation or mechanical entrapment. Here, strengthening is particularly important. Exercises that combine neural mobilizations (neurodynamics) with progressive muscle strengthening can help reduce neural sensitivity and improve functional capacity. This dual approach has been shown to be more effective than passive treatments alone.

Patients with Tension-Type Headaches and Migraines

Recent studies suggest that cervical muscle rehabilitation may reduce the frequency and intensity of tension-type headaches. Although migraines are more complex, with neurological and vascular components, some patients report improvements when muscular tension in the neck is addressed. A 12-week structured program that includes deep cervical flexor training, scapular strengthening, and posture education can yield measurable benefits in this population.

Patients with Temporomandibular Disorders (TMD)

Patients with TMD often present with cervical dysfunctions. Although the causal relationship is still debated, there is increasing evidence supporting the idea that the cervical spine and temporomandibular joint (TMJ) influence one another biomechanically and neurologically. Incorporating deep cervical strengthening into the treatment of TMD can improve posture, reduce referred pain, and contribute to better overall motor control.

Patients with Whiplash or Post-Traumatic Neck Pain

In cases of whiplash or cervical sprain/strain, the trauma often leads to a loss of cervical proprioception, weakness, and altered muscle recruitment. Strengthening protocols after such injuries are critical for restoring function and preventing long-term disability. In these patients, the therapeutic plan usually starts with gentle isometric work and progresses to dynamic exercises as tolerated.

Patients with Cervicogenic Headaches

Cervicogenic headache originates from cervical structures, often involving dysfunction of the upper cervical joints and muscles. Research supports the use of specific strengthening exercises combined with manual therapy to reduce headache intensity and improve cervical function. These exercises should include deep neck flexor activation, scapular retraction, and mobility exercises.

How to Strengthen the Neck Properly

General Strategy

An effective strengthening strategy is built on three pillars:

Scapular and Shoulder Girdle Strengthening: This offloads the cervical spine and helps stabilize the entire upper quadrant. Exercises may include rows, scapular retractions, and resisted arm movements.

Cervical Retraction Exercises: These target posture correction and reduce anterior shear forces on the cervical discs. The “chin tuck” exercise is a common starting point, often performed in lying and then progressed to seated or standing.

Deep Cervical Muscle Activation: Low-load endurance training of the deep neck flexors is vital. These muscles are often underactive in patients with pain and require targeted rehabilitation through biofeedback or controlled exercises such as the cranio-cervical flexion test.

Adaptation to Specific Conditions

Exercise programs must be adapted to the clinical presentation and goals of the patient:

In cervicobrachial pain, care should be taken to avoid positions that exacerbate symptoms. Combining neural mobilizations with isometric work may provide a safe entry point.

In headache patients, exercises should be integrated with trigger management, relaxation techniques, and possibly aerobic training.

In postural syndromes, strengthening must be accompanied by ergonomic advice and daily habit changes.

Dosage and Progression

The FITT principle—Frequency, Intensity, Type, and Time—guides safe and effective prescription. Initially, exercises may be done daily with low resistance and a focus on control. As the patient progresses, resistance can be added, and frequency can be adjusted to allow recovery. Endurance is typically more important than raw strength, especially in postural muscles.

A gradual increase in complexity—from isometric to dynamic, from simple to functional—is recommended. Patients should always be monitored for compensatory strategies such as overactivation of superficial muscles or altered breathing patterns.

Additional Considerations

While strength training forms the core of cervical rehabilitation, integrating aerobic activity has shown synergistic effects. Aerobic exercise improves blood flow, modulates pain perception, and promotes general well-being. It is particularly effective in cases of non-specific pain and should be encouraged as part of a global health strategy.

Conclusion

Strengthening the neck is a safe, evidence-based, and effective intervention for a wide range of conditions, including cervicalgia, cervicobrachial pain, tension-type headaches, TMD, and post-traumatic syndromes. It addresses underlying muscular dysfunction, improves posture, and empowers patients to regain control of their movement and quality of life.

By adopting a structured and individualized strengthening approach, physiotherapists can provide lasting improvements that go beyond symptom relief. In this context, rehabilitation is not only about restoring function—it’s about giving patients the confidence to move, live, and thrive again.

Introduction Strengthening the neck in patients suffering from neck pain or related conditions has become a central focus in physiotherapy.
